Looking Under the Skin: The Continuous Glucose Monitor (CGM) Revolution

Imagine climbing a mountain where you could only check your altitude once every three months.

That is how most of us manage our blood sugar. We eat, we stress, we sleep badly or well, and every 90 days or so a lab report tells us how the whole stretch went. By then, whatever damage the spikes did is already done, and whatever progress we made is old news.

A continuous glucose monitor changes that completely. On a recent episode of Lessons from the Ketoverse, we dug into more than a year of real world CGM experience: what the device actually does, what it taught us, and why the lessons matter whether you are managing type 2 diabetes, trying to avoid a diagnosis, or simply curious what food is really doing to your body.

What a CGM actually is

A continuous glucose monitor is a small wearable disc, usually worn on the back of the arm, with a tiny filament that sits just under the skin. It measures glucose in the fluid beneath the skin around the clock and sends the readings to an app on your phone, with roughly a fifteen minute delay.

Compare that to the traditional finger stick: a single snapshot, some discomfort, and a strong incentive to check less often than you should. The CGM gives you the whole movie instead of a few photographs. You see the rise after a meal, whether it settles back down within a couple of hours the way a healthy response should, and the slow trends that a quarterly lab test can only hint at.

One practical honesty note: CGMs are very good, but the finger stick remains the more precise measurement. Readings can lag when your glucose is rising or falling quickly, so it is smart to keep a standard meter on hand to verify anything surprising.

The food lessons come fast

Wear a CGM for even two weeks and the education begins. Sugary and ultra processed foods produce sharp, fast spikes, and your body does not care whether the sugar came from a candy bar or a natural source. Meanwhile, a plate built around protein and healthy fats barely moves the line. Watching your own steady curve after a satiating low carb meal is one of the most reassuring sights on this journey, because it is your body confirming the path in real time.

The surprises are personal, though, and that is the real power. One person handles blueberries beautifully and spikes on watermelon. Another reacts to a food they always assumed was safe. Hidden sugars reveal themselves within the hour. Even the order you eat in matters: starting with salad or vegetables, then protein and fat, and leaving anything sweet for last can meaningfully blunt the spike from the same meal.

We call the end result a designer diet: a way of eating engineered to your body's actual responses, not to someone else's template.

The biggest surprise is not food at all

Here is the lesson that shocks almost everyone who wears one of these: stress and sleep move blood sugar just as surely as food does.

When you are stressed, your body releases cortisol, and cortisol tells the liver to flood the bloodstream with glucose. That response was brilliant when the stressor was something you had to outrun. It is much less helpful when the stressor is a difficult email, a rough night of sleep, or a near miss on a ladder. One of us watched his baseline glucose double within twenty minutes of a genuinely frightening moment during a construction project, with no food involved at all.

The takeaway is important for anyone changing their diet: if your stress is chronic and your sleep is poor, food alone may not get you across the finish line. The CGM makes that visible, and it also shows the fix working. Time in nature, a quiet walk, mindfulness practice: you can literally watch your glucose settle as your nervous system does.

The single healthiest habit the data taught us

If we had to name one practical habit that a CGM proves beyond argument, it is this: walk after you eat.

Ten to twenty minutes of easy walking after a meal visibly pulls glucose down as your muscles put it to use. Catch an unexpected spike, maybe from a hidden sugar or a stressful afternoon, and instead of lying down and letting it linger, you take a walk or hop on the elliptical and watch the line bend back toward normal. It turns you from a passenger into a driver.

Getting started, and getting good data

A few practical notes from a year of daily wear. Talk to your healthcare provider first, especially if you have a diabetes diagnosis, since coverage often depends on it and your care team should know what you are doing. Expect a cost of roughly $120 Canadian per two week sensor if you are paying out of pocket, which sounds like a lot until you weigh it against what poor metabolic health costs.

On the practical side: the sensors are designed for the fattier tissue on the back of the arm, and leaner users may need to experiment with placement. Rotate sides with each new sensor so your skin gets a break, and if sensors tend to fall off during workouts, a clear adhesive patch over top solves it. Log your meals in the app honestly, because the data is only as useful as the notes you attach to it. And do not become obsessive. The goal is insight, not a new source of anxiety. Learn your patterns, make your changes, and let the device fade into the background.

One important caution: if you take diabetes medication, never adjust it based on CGM readings without your doctor. The data is for learning and conversation, not for self prescribing.

Do you need to be diabetic to benefit?

In our view, no. Neither of us is sponsored by any CGM company, and we will happily say the devices are not perfect. But the case for wearing one even temporarily is simple: two weeks of data will teach you more about how food, stress and sleep affect your body than years of guessing. Many people avoid measuring because they are afraid of what they will find. We understand that fear, and we have lived the alternative, which is finding out years later at a doctor's office when the trend has already done its damage.

The worst outcome of looking under the skin is that you get healthier.
